    • Information from adoption record including name of birth mother and name of birth father if recorded.
    • Certificate allowing access to original birth record.
    • Undertaking if contact veto.
    • May register contact veto.
    • May record wishes about meeting.

    ADOPTED PERSON UNDER 18 YEARS ...
    (need written permission to apply from adoptive parents)
    • Non-identifying information from adoption record.
    • Identity of birth mother and birth father if recorded, with writen agreement of birth mother / father.
    • Certificate allowing access to pre-adoption birth record with written agreement birth mother/father.
    • Undertaking if contact veto.
    • May register contact veto.
    • May record wishes about meeting.
      FEE: $A75.00

    REGISTRANT STATUS - BIRTH PARENT / NATURAL RELATIVE:
    ADOPTED PERSON 18 YEARS AND OVER

    • Information from record of adoption, including name of adopted person and surname of adoptive parents.
    • Undertaking if contact veto recorded.
    • May register contact veto.
    • May record wishes about meeting.
